WHAT YOU WILL DO:
The Elder Services Case Manager is an exempt position responsible for the direct supervision of case managers and case management services in the Community Care for the Elderly (CCE) Program.
- Day-to-day supervision of CCE Lead Agency case managers in accordance with requirements as established by funding sources, including staff recruitment, meetings, and training.
- Supervise personnel in the field through individual conferences and timely evaluation of employees’ performance.
- Assign clients to case managers.
- Manage client service costs as required by the funder, including but not limited to review and approval of One Time Only (OTO) requests to the funder.
- Provide information and referral for appropriate ancillary support services.
- Assist with recruiting, interviewing, and hiring in the absence of the Assistant Program Director
- Maintain a case load of up to 30 clients and in the absence of an assigned case manager, coordinate case management and support services, including Adult Protective Services (APS) High Risk Referrals
- Complete required reports, including outcome measure reports and narrative reports, as required by the funding agency.
- Appropriately manage peer file reviews and scheduled file monitoring for file integrity as it pertains to the Department of Elder Affairs (DOEA) service manual on an ongoing basis, review, revise, and correct documentation as it pertains to billable and non-billable activity and provide quality assurance training to all Elder Services Department staff to ensure continuity of file integrity.

WHAT YOU WILL NEED:
- Bachelor’s degree in social work, sociology, psychology, or related social services field
- Bachelor’s degree in a field other than social services field with professional human services experience may substitute on a year-for-year basis for education requirement, if approved by funder
- Two (2) years of case management experience
- Thorough knowledge of department programs and functions.
- Considerable knowledge of community organization and resources
- Ability to communicate effectively, including in a supervisory capacity
- Competent in using Microsoft Office applications, particularly Word and Excel
- Knowledge of the management budget system is preferred
- Must successfully complete 701b Assessment training and receive certification
- Must have a valid driver’s license and current auto insurance coverage.
