Service Coordinator

Location: Tampa, FL

WHAT YOU WILL DO:

The Service Coordinator is a non-exempt position responsible for general service management at the Myrtle Oaks Apartments, a supportive housing program for the elderly.

  • Provides general service management which includes intake assessment, education (services available and application procedures) and referral of residents to serve providers in the general community. These social services may include meals-on-wheels, transportation, home health aides, homemakers, financial assistance, counseling, preventative health screening, and other needed services
  • Develop and update as necessary a Resource Directory. This Directory may include a listing of state and/or local service providers that residents can contact for assistance (e.g. services to families, children, individuals who are elderly, persons with disabilities, emergency assistance). In many cases, state and local governments can also provide a listing of the non-profit agencies with which they contract for services
  • Coordinate educational events which may include subjects relating to health care, agency support, life skills, referral sources, etc.
  • Facilitates the formation and maintenance of Resident Associations within the property if the residents are interested. The formation of these groups assists the residents in planning social events, organizing activities and discussing daily life issues. Assures Resident Association structure is in compliance with By-Laws
  • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate
  • Serves as a liaison to community agencies, networks with community providers and seeks out new services available to the residents
  • Assists the residents in building informal support networks with other residents, family and friends
  • Complies at all guidelines in the HUD Service Coordinator Resource Manual, including the timely completion of report and emphasis on client outcomes
  • Perform all other duties as assigned

WHAT YOU WILL NEED:

  • Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable.
  • Minimum of two (2) years' experience in social service delivery for seniors and/or non-elderly disabled. Supervisory experience may be necessary at some capacity.
  • Demonstrate a working knowledge of supportive services and other resources in the area served by the project
  • Demonstrate the ability to advocate, organize, problem-solve and provide results for the residents they serve.
  • Demonstrate the capability to meet the standards of federal reporting requirements.
  • Bilingual, required in languages most spoken by clients (English/Spanish)
  • Must complete and obtain HUD Service Coordinator Certification within 12 months of employment
  • Must have a valid drivers’ license with auto insurance coverage
