Substance Abuse Case Manager

Location: Clearwater, FL

WHAT YOU WILL DO:

The Substance Abuse Case Manager is a non-exempt position responsible for providing services to individuals diagnosed with substance use disorders within the behavioral health continuum to include both Supported Housing and Intervention services.

  • Respond to referrals in the Intervention Program and evaluate referred individuals for program appropriateness.
  • Maintain a split caseload consisting of community-based clients and provide linkage to the necessary support services.
  • Provide in-home intervention and education sessions related to substance abuse issues.
  • Prepare assessments and assist with locating housing and/or linkage to the appropriate level of care.
  • Collaborate with clients utilizing education, motivational interviewing techniques, focused discussions, advocacy, supportive counseling, and problem resolution.
  • Coordinate the service planning process with the primary goal of assisting the client with achieving independent living.
  • Provide information and linkage to community resources.
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:

  • Bachelor's degree in human services or related field
  • One (1) year experience working with substance abuse.
  • Must obtain/maintain CPR and First-Aid certification within 90 days of employment
  • Proficient in Microsoft Office Suite and Electronic Health Records platforms
  • Must have a valid drivers' license with auto insurance coverage.
