WHAT YOU WILL DO:

The Mentor is non-exempt position responsible for supporting individuals in the fatherhood program to serve in a mentor role. The Mentor work in conjunction with trained case- managers to provide outreach, engagement, and referrals.

• Provide in person response to referrals within contractual time frames.
• Initiate outreach and provide ongoing engagement with individuals experiencing overdose to build a trusting, peer-based relationship.
• Encourage clients to follow through with referrals and recommended follow-up for parenting classes.
• Provide information and linkage to community resources.
• Perform all other duties assigned by Program Manager

WHAT YOU NEED:

• High School Diploma or GED equivalent
• Provide high-level encouragement and guidance support for routine and day-to-day work.
• Proficient in Microsoft Office Suites and Electronic Health Records
• Excellent verbal and written communication skills
• Must have a valid drivers' license with proof of auto insurance coverage.
