WHAT YOU WILL DO:
The Life Coach is a non-exempt position responsible for providing support to youth and young adults ages 16-24 to maximize their likelihood of making a healthy transition into adulthood.
- Establish a relationship with the client, family/guardian, significant other, and all other service providers connected to the client
- Conducting strength-based needs assessments related to the interests, values, goals, and aspirations of clients.
- Coordinate the service planning process, with the primary goal of assisting clients with achieving financial independence.
- Participate in client staffing and follow-ups as assigned.
- Identify, deliver, create, broker, and link together the needed supports and services across the transition domains of employment, education, living situation, and community-life functioning for clients.
- Maintain an active caseload of community clients and link these clients with the necessary community support services to maintain the individual.
- Must ensure concise and timely completion of all paperwork or documentation, including but not limited to, administrative, clinical, and/or caseload requirements.
- Attempts to generate change and modification in the client’s behavior and attitude to facilitate personal growth, independence, and recovery.
- Perform other duties as assigned.

WHAT YOU WILL NEED:
- Bachelor's degree in human services, psychology, social work, sociology, criminology, or any related field
- Previous work history in the behavioral health field with adolescents and young adults preferred.
- Experience working with youth or young adults coping with trauma and behavioral health issues and homelessness strongly preferred.
- Attend and complete Transition to Independence Process (TIP) Model Core Competency Training.
- Must obtain/maintain CPR/First Aid certification.
- Competency with Co-Occurring disorders.
- Working knowledge of community organizations and resources.
- Proficient with Microsoft Office applications, particularly Microsoft Word.
- Effective verbal and written communication skills.
- Strong organizational skills and attention to detail.
- Ability to conduct formal and informal meetings.
- Must be available on call (24 hours) to handle a crisis either in person or by phone, depending on the nature of the crisis.
- Ability to reach, bend, crouch, and climb stairs while assisting with life skills in consumers' homes.
- Ability to work in the field at any given time, season, and climate in both indoor and outdoor environments.
- Ability to work overtime when needed.
- Required to use personal car to transport clients as needed. Must have a valid driver’s license with auto insurance coverage.
