Life Coach

Location: Oakland Park, FL

WHAT YOU WILL DO:

The Life Coach is a non-exempt position responsible for providing support to youth and young adults to maximize the likelihood of a healthy transition into adulthood. The Life Coach works with individual youth and young adults to mentor and teach life skills in various settings, including the youth/young adults’ home and in the community.

  • Establish a relationship with the youth/young adult, guardian, significant other, and all other service providers connected to the youth/young adult
  • Conduct assessments and pre- and post-testing with youth/young adults
  • Identify, deliver, create, broker, and link together the needed supports and services across the transition domains of employment, education, living situation, and community-life functioning for consumers
  • Provide life skills teaching and hands-on coaching to ensure youth/young adults are meeting their independent living goals as identified in their independent living plan
  • Develop an independent living plan with the youth/young adult, including goals and specific plans with time frames
  • Maintain an active caseload consisting of youth/young adults in the community and link these youth/young adults with the necessary community support services required for maintaining the individual
  • Transport youth/young adults to activities/services in the community
  • Provide psychoeducational groups on various life skill topics
  • Ensure concise and timely completion of all paperwork or documentation, including caseload requirements
  • Provide secondary family support as needed
  • Perform other duties as assigned

WHAT YOU WILL NEED:

  • Bachelor's degree in human services or related field? and one (1) year working with youth or relevant case management experience
  • Experience working with youth or young adults coping with trauma and behavioral health issues, and homelessness is strongly preferred
  • Working knowledge of community organizations and resources?
  • Must possess the ability to operate Microsoft Office applications, particularly Microsoft Word, Outlook, and Teams
  • Must possess effective verbal and written communication skills
  • Detail and organizational skills are critical
  • Ability to conduct formal and informal meetings
  • Must be available to handle crisis either in person or by phone
  • Ability to reach, bend, crouch, and climb stairs while assisting with life skills in youth/young adults' homes
  • Ability to work in the field at any given time, season, and climate in an indoor and outdoor environments
  • Ability to use of personal vehicle to transport clients as needed.
  • Must have a valid driver’s license and proof of auto insurance coverage.
