Family Care and Wellness Coordinator - Part Time

WHAT YOU WILL DO:

The Family Care and Wellness Program Coordinator is a non-exempt position responsible for coordinating accessible, family-centered services for parents, caregivers, and children in Hillsborough County from birth through high school age who have mental health or wellness needs. Working in partnership with families and the program team, the coordinator manages referral intake and screening, maintains accurate documentation, submits referrals, and follows up on other support services. Through responsive coordination, this role strengthens protective factors, promotes family stability and well-being, and helps prevent involvement in the dependency system.

  • Manage referrals from initial receipt through screening, eligibility review, assignment, and connection to appropriate program or community-based services.
  • Engage parents and caregivers in identifying family strengths, needs, priorities, and barriers to accessing mental health and wellness supports.
  • Maintain timely, accurate, and confidential records of screenings, referrals, family contacts, services provided, follow-up activities, and outcomes in accordance with agency and funding requirements.
  • Participate in case consultations, team meetings, outreach activities, and cross-agency coordination to support continuity of care and strengthen community partnerships.
  • Assist with program data collection, reporting, quality-improvement activities, and compliance with organizational policies, contractual requirements, and applicable regulations.
  • Assist with office filing and documentation for case files.
  • Assist Program Manager with ensuring that Wellness Specialists have access to helpful activities, forms, and handouts that can be provided to caregivers and children.
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:

  • High School Diploma or equivalency, and at least two (2) years of experience in the human services field.
    • Bachelor’s degree preferred.
  • Demonstrated ability to coordinate referrals, track follow-up activities, and manage multiple priorities within established deadlines.
  • Strong organizational and administrative skills, including accurate recordkeeping, file maintenance, scheduling, and data entry.
  • Effective verbal and written communication skills, including the ability to explain services clearly, document family needs, and prepare concise reports.
  • Experience working with children to better communicate with families to support staff in families reaching goals.
  • Ability to protect confidential information and comply with agency policies, funding requirements, and applicable privacy standards.
  • Proficiency with Microsoft Office, electronic records, databases, and other systems used for documentation and communication.
  • Ability to accept and work comfortably with individuals of different races, nationalities, religions, cultural patterns, and standards of living.
  • Ability to work under stress
  • May be required to use a personal vehicle to transport clients or complete other work-related travel, depending on program and client needs.
  • Potential exposure to clients who may be verbally and/or physically aggressive.
  • Potential exposure to communicable diseases.
  • Must have a valid driver's license with auto insurance coverage.
